Below are our Employment Application and Background Check Authorization Form. Please print both forms and fill them out and we can get you all started.

If you prefer to print it out and bring into the office at the time of your interview or Download; the PDF above then, open each form, fill out and sign (type) your signature where required, save and email the forms back to Angela Maye at Compassionate Hands In Home Agency = Chief Compliance Officer=

You will also need to bring a copy of your CPR Certification, driver’s license, and social security number card, proof of citizenship, proof to work in the US to run your background check and employment verification as a state compliance. If you do not have your CPR Certification, let us know and we can help you get them.
